Methodology hardware-controlled bipolar electrocoagulation in the surgical treatment of chronic hemorrhoids combined Department of surgical diseases with the course of Coloproctology State Medical University.Acad.IPPavlov and St. Petersburg City coloproctological center uses more than two years.data were found in the literature about the use of this technique in patients with acute hemorrhoid.
Initially, at the stage of development of methods of hardware-controlled bipolar electrocoagulation, acute hemorrhoids is a contraindication to its use.But the use of it during surgery for chronic hemorrhoids under local infiltration anesthesia showed that the saturated solution of local anesthetic tissue in the surgery model edematous tissue haemorrhoid thrombosis and acute hemorrhoids.
The purpose of the study. purpose of this study is to evaluate the possibility of using hemorrhoidectomy using a hardware-controlled bipolar electrocoagulation in patients with acute hemorrhoids II - III stages.
Materials and Methods.Since December 2003, the St. Petersburg city center coloproctological were operated on 10 patients with acute hemorrhoids stage II on the International Classification of using a hardware-controlled bipolar elektrkoagulyatsii.
All patients of the study group suffered from acute hemorrhoids in the form of thrombosis of external and internal hemorrhoids with inflammation of the perianal skin.
The disease from the beginning of the last attack of acute hemorrhoids prior to surgery ranged from 12 hours to 4 days.Four patients received preoperative anti-inflammatory therapy - heparin ointment locally.
The group evaluated: duration of surgery, bleeding during surgery, pain (using an international visual analogue scale and the need for the appointment of narcotic analgesics in the postoperative period), the presence and severity of postoperative complications: dizuricheskih disorders, post-operative bleeding, recovery timesdisability of patients, post-operative healing time of wounds . To investigate the effect of a technique of surgery on the function of the sphincter apparatus of the rectum and anal canal in patients in the postoperative period was carried out in the domestic profilometry apparatus "Kolodinamik-3".
Results: When using hardware-controlled bipolar electrocoagulation noted the almost complete absence of bleeding during surgery and in the postoperative period.A significant reduction in postoperative pain, lack dizuricheskih disorders, rapid recovery of patients disability associated with bipolar electrocautery gentle influence on the fabric.Considerably reduce the time of hospitalization. The average hospital stay was 6.5 days.
Conclusion. In assessing hemorrhoidectomy techniques provided data on the significant decrease of post-operative pain, a significant reduction in the number of postoperative complications and reduce the time of surgical intervention in patients with acute hemorrhoids II - III stage of the international classification using the procedure of hardware-controlled bipolar electrocoagulation.
